Informational Banner at the top of each order page

Would love to have a box/ informational banner at the top or maybe even bottom of each order form where we can add additional information such phone number, email, and maybe links to our actual website.

Also in the same box where we can add side notes such as: (ex: for same day booking, contact us!)(ex:2 use code: feb20 for 20% off all February!)
