Travel Fee, Price Per Mile after Service Area

We need to have the ability to set a service area AND a travel fee based on per mile after a certain threshold, and have it customizable with different thresholds.
Example:
0-45 miles - $0 (In Service Area)
45-60 miles - $1.50 per mile

60-100 miles - $3.00 per mile
